If you cannot come to us in our bespoke clinic, we may be able to come to you depending on your geographical location. We match you with OTs who can work with you in the comfort of your own home so you can get the best out of your treatment in your own environment. Our practitioners are well trained and carry all the necessary equipment, skills, and resources to conduct multiple assessments. We also serve DVA and NDIS clients and we write second opinion OT report if you wish to seek second opinion on your existing OT report. Everyday, our OT Pauline spends most of her time to deliver results and to be the Nexus between hope for a wheelchair, and reality of having that wheelchair delivered; the Nexus between hope to be more mobile, and the reality of moving around one’s home and driving a wheelchair that is specifically tailored to the client’s needs. Mental Health and Wheelchair Expert OT Pauline just delivered this Jay Fusion Magic 360 Power Wheelchair in record time.

process for a scooter application Of an Agency Managed NDIS patient

Once Pauline determined that our client’s scooter was above $1500 and not approved by NDIA, she undertook the following process:

Discussioin

She discussed the scooter, its specifications and tailored modifications and researched the right scooter to fit the client’s needs.

Submit AT Application

Pauline then sent the AT application to the NDIS via secure email.

Fix Trial Date

Pauline understood that some higher cost items cannot be returned once ordered so she planned a trial date with her client and the supplier.

NDIS Reply

Pauline advised our patient that application had been submitted and that the patient will be informed by NDIA when the application is decided.

NDIS Appllication

Pauline’s second trial was successful so she completed an “NDIS Assistive Technology General AT Application Form”

Approval *

On this occasion, our client’s scooter was approved so Pauline followed up and requested delivery.
*Applications are not always successful.

Sign Consent

She then sent this form to the participant to sign consent for the Application to proceed to the Agency.

Delivery

She then attended the client’s home and oversaw the setup and educated the client on how to safely use, store, maintain, troubleshoot and  care for their scooter.

We offer in home and aged care, one on one consults. Our OTs are experienced in providing services under the Home Care Packages and DVA. All our OTs are Medicare registered. We understand the importance of being able to access therapy services in the comfort of your own home. Our Occupational Therapists (OTs) are skilled in maximizing your independence and working with you to achieve your functional goals, whether it’s improving mobility, enhancing self-care skills, or living independently.
